TO PROPERLY PROCESS YOUR WARRANTY CLAIM FOR ANY MANUFACTURED PARTS BY DYNASTY SPAS ALL FIELDS MUST BE FILLED IN ON FORM • PLEASE ATTACH TWO PICTURES
PLEASE PROVIDE A PICTURE OF THE DEFECTIVE PART AND A PICTURE OF THE SERIAL NUMBER
HOW TO FILE A PUMP CLAIM
1. Complete the form below.
2. Attach the physical pump labels paper.
3. Include the claim ID number which will be generated on the Thank You receipt after your fill out the form below.
4. MUST Mail-In the defective PUMP LABELS to:

For all other defective parts, pictures of the defective part must be uploaded.
Returning the physical part is not required

Please take two pictures one of the part itself and the serial number label circled in red and upload picture with the filled out form below to process your claim much quicker and smoother.

Jets, Valves, Plumbing, Diverters, LED light strands, Electrical cables, Jet bodies, Wall fittings, etc.
Two pictures of the complete part showing the defect is required.
